- Ask the signer to use the sample wording to prepare a letter on employer or organization letterhead. Replace all bracketed fields and remove the specimen heading and example-only note.
- The letter must specifically verify your competency in peripheral IV (PIV) placement and the care of PICCs and central venous catheters. The signer must retain only skills they personally observed and verified, and remove any statements they cannot attest to. The sample lists additional skills; it is not a requirement to attest to all of them.
- Include your full name and nursing credentials, plus the signer’s name, credentials, role, organization, professional email and phone number. Have the signer sign and date the letter.
- Save the completed letter as a PDF and upload it under IV competency/training letter in your document checklist. Do not upload the blank sample or include patient information.
ABC will review the completed letter. Downloading the sample does not complete your IV requirement. If you have neither an IV certification nor a letter verifying the required skills, you will need to complete an IV certification course before submitting your application.
